Novelist Jiro Yotsuya is admitted to a remote hospital in Tokizaka after a bus accident, where he enters a volatile relationship with the enigmatic and sexually aggressive Yu Yagami. As Yu's deep-seated trauma and insanity surface, the hospital transforms into a slaughterhouse, forcing Jiro to fight for his life. Years later, having found fame by writing about the horror, Jiro discovers that the nightmare he escaped may not be over.
